Transcript Request Form

The official Transcript of Records is not automatically sent when you have completed your courses - you need to fill out the form below when the final grade has been registered. 

The official transcript will only include grades from courses found under completed courses at My LTU > Results > Registry Transcript.

Please make sure that all your courses are completed before you apply for your Transcript.

We send the official transcript via postal services which can take up to two weeks. The transcript is sent to the address given in the form. If you want the transcript sent to your home university, please write the contact person or office it should be addressed to.
