To supervisors and student affairs staff - the information in the template below should be used when obtaining consent for the processing of sensitive personal data and can also be used on other data. Text in bold should be adjusted according to specific conditions.

Student affairs staff should check the final version of the text with their supervisor before sending it out to participants. The supervisor can be supported by the legal service in questions related to handling and drafting of the text. See also the separate information sheet, which should be provided to all participants before the study. This document can also be signed digitally with eduSign.
