Notification of incident Employee

An incident is an undesired event linked to the physical work environment, threats and violence, fire, health problems linked to the indoor environment or the psychosocial work environment that could have resulted in personal injury or ill health.

How to do:

As a meber of staff you shall always report the incident through self-reporting in Primula. You do this when you have been in place when the incident occurred or as soon as you discover that something has occurred that could have resulted in personal injury or ill health. The report is sent via Primula to the immediate supervisor who is responsible for investigating the incident together with you and the local safety representative. If a local safety representative is not available, contact the primary safety representative.

Report serious icidents to the Swedish Work Environment Authority

If an event could have resulted in extensive personal injury or ill-health, the incident is serious. An example of a serious incident is, an explosion in a currently empty lab or threat of violence or life threatening which is perceived as serious. The immediate supervisor is responsible for reporting serious incidents to the Swedish Work Environment Authority.

Always save a copy

The form is automatically sent to the Swedish Work Environment Authority. Print the receipt of the notification and always send a copy to the HR unit. Also keep in mind that the safety representative and the notifier must have a copy of the receipt
